Does a HIPAA Authorization Need to Be Notarized in Kentucky?

What does federal HIPAA law actually require on an authorization?

A valid HIPAA authorization needs specific written elements, defined by the HHS authorization guidance: a description of the information to be released, who may release it, who receives it, a purpose, an expiration date or event, the patient's signature and date, and statements about the right to revoke. Notice what is absent: no witness requirement, no notary requirement. A provider cannot refuse to honor an otherwise valid authorization *solely* because federal law demanded a seal, because federal law never did.
